By using our website or opting in to receive text messages from us, you agree to these Terms. Information only: Content on this website is for general information and is not a substitute for an examination, diagnosis, or advice from a dentist. Using this website does not create a dentist-patient relationship.

Appointments, Patient Forms and Contact Forms
Appointment requests made through our website are not confirmed until our office contacts you. Please do not include detailed health information in general contact forms; use our patient forms or share it securely during your visit. Please provide accurate contact information so we can reach you.
Text Messaging (SMS) Terms
Program description: Midland Dental and Oasis Kids Dental and Orthodontics send text messages to patients, parents and guardians, and prospective patients who have agreed to receive them. Messages may include appointment reminders and confirmations, recall (checkup) reminders, requests to complete patient forms, billing and payment notices, requests for feedback about your visit, and replies to messages you send us.
How you opt in: You agree to receive text messages when you provide your mobile number to our office (in person, by phone, on patient forms, or through our website) and consent to receive texts, or when you text our office first. Consent to receive text messages is not a condition of receiving care or making a purchase.
Message frequency: Message frequency varies based on your appointments and account activity.
Costs: Message and data rates may apply. Contact your wireless provider for details about your plan.
Opting out: You can cancel at any time. Reply STOP to any message to stop receiving texts. You will receive one final message confirming that you have been unsubscribed. To rejoin, reply START or contact our office.

Carrier disclaimer: Wireless carriers are not liable for delayed or undelivered messages. Delivery is subject to effective transmission from your wireless provider.
Eligibility: You must be the account holder or authorized user of the mobile number you provide. For patients under 18, messages are sent to the parent or guardian who provided consent.
Privacy: We do not sell or share your mobile number or text messaging consent with third parties or affiliates for marketing or promotional purposes. See our Notice of Privacy Practices for details.
Payments and Dental Savings Plan
Fees for dental services are discussed with you at your visit. Online bill payment and financing, where offered, may be handled by third-party providers that have their own terms and privacy policies. Our in-office Dental Savings Plan is governed by the plan terms provided when you enroll. Questions about your account can be directed to our office.
Intellectual Property
The text, images, logos, and other content on this website belong to Midland Dental and Oasis Kids Dental and Orthodontics or their licensors and may not be copied or used without our permission.
